Seems like some manufacturers design their cars to allow exhaust gases to be sucked back into the engine in order to reduce nitrous oxide emissions. The issue is under certain circumstances debris from the catalytic converter, which breaks up due to age and/or unburnt fuel coming into contact with a very hot catalyst, being also sucked back into the engine that results in scouring and damaging pistons, bores etc.
Apparently there is no warning that the above is happening, and the only reliable solution is to either replace the cats or de-cat.
