Coolant top up?

Has anyone ever had to top up their coolant before? I always thought if it was needed then it indicated a leak somewhere..? Or maybe the factory never filled them correctly? (as mine is reading at the minimum level when warm: 2005 with 15k on the clock)
Should I be concerned?

Never filled it up in 65k miles.

22K and still at the correct level.

I'm guessing it would need renewing at some point though. 15 litres I believe.
